Medical coordinator of the coronary unit of Clínica Cuyo

Born in La Paz, the Argentine Ariel Baigorria Jayat has been a medical coordinator at the coronary unit of Clínica Cuyo since 2006. For two years, he has also coordinated the cardiovascular recovery unit at the same center. In addition, he is the vice president of the Cardiology Society of Mendoza and a very active member of the Argentine Federation of Cardiology (FAC), where he has developed an intense scientific activity. This year, he was assigned vice president of the scientific committee of the National Cardiology Congress of the FAC. Regarding Practicum Script, he considers the simulator to be "THE tool to reduce medical error."

Madrid - September 11, 2018. What are the advantages of the Practicum Script (PS) course for your training and exercise as a specialist?
It is a good way to approach the patient. In addition, it is useful to keep you updated.

Why is differential diagnosis important?
I consider it key during the elaboration of a presumptive diagnosis since it reduces the possibility of mistakes. From my perspective, diagnostic error is a capital error because it leads to everything else being wrong.

Practicum Script addresses dilemmatic and controversial situations. To what extent is it interesting to discuss results or considerations with other colleagues and with the tutor?
It is always interesting to know how other colleagues think. Discussions with them make you learn.

Do you think that clinical reasoning training improves patient care? Have you noticed any change in your way of reasoning a diagnosis?
I believe without a doubt that it improves patient care. In the consultation room, when analyzing an incoming patient, I noticed significant and useful changes in my entire team.

What do you think about continuous training and evaluation of competences?
I consider continuing medical education to be mandatory in professional practice. As Favaloro said, "one Sunday that I did not read, I fell behind."

Does Practicum Script help reduce medical error?
I don’t know the figures in my country. In my center, management exercises are carried out. This form of reasoning of PS is THE tool to reduce medical error.

In what context do you practice your specialty and to what extent does the simulator help you given these conditions?
I am the critical cardiology coordinator at my workplace. When analyzing an incoming patient, we put into practice the methodology of possible diagnoses, pros and cons, and the selection of complementary tests and appropriate therapies. It is even useful for the auditors (I work in a private center) in supporting our decisions.

Cardiovascular disease is the main cause of mortality in high-income countries, and is also showing a continuous increase in low- and middle-income countries. How do you deal with this reality?
With concern, trying to reinforce prevention measures in patients, groups of friends and family.

For me, the program is very interesting, since it allows me to sharpen my interrogation and direct it more towards a specific pathology and to have more knowledge of laboratory and cabinet exams.
