The Mexican doctor Marisol Fonseca is a pediatrician specialized in Intensive Therapy. She was born in 1982, graduated from the Anahuac University, and was trained in her subspecialty at UNAM. She worked in 2013 at the Boston Children's Hospital and is currently working at the Hospital de Quemados de Puebla. Among the merits attributed to Practicum Script, she mentions the reinforcement of analytical skills and clinical judgment, broadening of differential diagnoses, and the possibility to review (cases) and stay continuously up to date. Her observations on the simulator emphasize its value in "integrating the pathologies more critically and efficiently."
Madrid, October 23, 2018. Considering your age, it has not been too long since you finished residency. Do you consider Practicum Script to be a useful ally for residents?
For me, it has been a good tool to continue practicing clinical analysis and judgment. Thanks to Practicum Script, I remember diseases that I have seen in General Pediatrics and other diseases that I no longer see.
Would you say that with more simulation you become more confident in your decision making in the presence of the patient?
It is easier to make decisions when you have practiced previously. With the simulator, I have learned to organize better my ideas; it has also helped me review some concepts.
Do you think that medical error rates correlate to gaps in clinical thinking skills?
I completely agree with that.
Your course indicators show an approximate 80% rate in coincidental hypotheses and scenarios. What do you think about comparing your results with those of a panel of experts?
Sometimes, my answers differ a little, but I am glad that most of my hypotheses coincide with those of the experts. This shows that there are similarities between Mexico and other countries.
How do you deal with dilemmatic situations without categorical evidence?
I believe that I analyze and evaluate them, and by ruling out (hypotheses), I arrive at a result.
What role does clinical expertise play in patients who fall outside the norm?
It only helps us to be more confident in making decisions, but we can make mistakes in our diagnoses.
You know that the cases are real. Do you actually observe a translation of what is presented in the simulation exercise into your care practice?
Yes, I see that it looks like real practice.
How is your work environment and what type of patients do you see? How does PS fit into your care practice?
I currently work only in the neonatal area, at a university and a public hospital. This platform helps me remember things that I no longer put into practice. I find it very useful because it helps me integrate pathologies more critically and efficiently.
Our base office is in Spain, but the digital transformation along with the present time allows the course to reach the state of Puebla in Mexico. What does this universal access mean for your professional practice?
I am grateful that things are like that because sometimes it is difficult to travel out of state. Having an online tool allows me to be (or to attempt to be) always up to date.
