A 2004 graduate from the National University of the Northeast, the Argentine Marcelo Javier de la Rosa is a cardiologist in Corrientes. Specifically, he practices in the town’s Cardiology Institute, in the area of cardiovascular rehabilitation. Dr. de la Rosa is convinced that "knowledge in medicine is essential but does not constitute 100% of the clinical practice, since we deal with patients with different complexities." This is his point of connection with the platform of simulation and debate of clinical cases, Practicum Script.
Madrid, January 8, 2018. We are accustomed to using simulation to improve motor skills, but what advantages do you see in (simulation) improving clinical reasoning?
Clinical reasoning is a fundamental link in medical practice. I think that simulation is an ideal form of practice, allowing the generation of a list of relevant differential diagnoses and the application of adequate diagnostic algorithms, allowing us to weigh each diagnosis in terms of relative probability. This, added to the fact that we can compare our knowledge with that of a panel of experts, definitely magnifies the validity of this tool.
What has Practicum Script specifically brought to you?
Practicum Script provides what is necessary for an organized while flexible mental blueprint about our everyday clinical cases, including others that do not fit so much in our clinical practice. In addition, it allows us to compare our knowledge with that of recognized physicians.
You know that uncertainty is this course’s principle of being. What do you think about that?
Without mentioning a particular case, I am generally very satisfied with the method used and the possibility of managing different possible scenarios in clinical practice. I am looking forward to the next course.
From the perspective of objectivism, knowledge is considered an end in itself and constitutes the only truth. How does Practicum Script break with this?
Knowledge in medicine is essential but does not constitute 100% of the clinical practice, since we deal with patients of different complexities. That is why common sense, the strength of this course, is vitally important.
In medicine, security is an invaluable intangible. Do you think that Practicum Script reinforces this culture?
Seeing different daily practice cases, concerning both diagnosis and treatment, and examining them against a panel of experts helps to improve safety, especially in terms of our knowledge and ability to reason different clinical scenarios, taking into account a humanistic and individualized vision of each patient.
